*AS IS STANDARD IN HEALTH CARE, APPOINTMENTS MUST BE IN THE NAME OF THE CLIENT, THE PERSON RECEIVING TREATMENT. (The Guardian must accompany the client if required.)
* Massage Therapy isn't emergency care. Please seek appropriate medical advice in the case of emergency or acute injury.
*NEW AND MANY RETURNING CLIENTS: You will be required to fill out & sign new online forms and documents to have your appointment confirmed. Look for emails for the forms you need to complete, sign & submit online. ***Please alert the therapist if you're unable to access these forms, accommodations can be made.
PAYMENT: Due prior to or at time of treatment. No deferred payments allowed. Payment options in office are credit card, various touchless/tap pay services, HSA and cash. (Cash must be exact. Therapist won't be able to make change. No Checks, FSA or HRA cards or insurance billing.) **Clients always have the option for electronic receipts. For privacy, receipts do not include treatment details.
